Waihi Underground Explosive Principal Hazard Management Plan Approved by: SSE/ General Manager Approval Date:11/05/2021 Next Review: 08/04/2022 OceanaGold Waihi Document ID: WAI-400-PLN-007 Page 11 of 27 4.3 Underground Mine Foreman It is the responsibility of the Underground Foreman for collating all of the documentation relating to blasting activities i.e. face sheets and blast number recording. In addition, ensure: • All necessary documentation is being completed and filed appropriately • All inspections and stock takes are completed and in accordance with this PHMP (section 5.6) • Any discrepancies are reported to the Underground Mining Manager as per the site procedures (section 5.6) • Compliance and maintenance of standards of all magazine rules (section 5.4) 4.4 Production Engineer The Production Engineers are responsible for: • Blast design and issue of approved charging plans (section 6.2.1) • Ensure drilling, charging and firing operations are conducted in accordance with approved charging plans • Compliance with all magazine rules (section 5.4) • Ensure magazine stock takes are completed in accordance with site procedures (section 5.6) • Explosives stock control 4.5 Underground Shift Supervisor The responsibilities of the Shift Supervisor in relation to explosives are: • Supervising handling, transport, charging, firing and re-entry operations and ensuring compliance with procedures and approved charging plans • Giving clearance to the Charge-Up Operator for blasting (section 6.4.4) • Giving re-entry clearance for headings after re-entry procedures have been completed by competent persons (section 6.5.2) • Ensure all persons who handle, transport, charge or fire are trained, assessed, ticketed and authorised to do so or under direct supervision of an authorised person (section 4.10) • Ensure completion of misfire book as required (section 6.8) • Communicate all charging and firing information to the oncoming Shift Supervisor • Ensure that priority is given to locating explosive product should a discrepancy be found during a stocktake. 4.6 Charge-Up Operator The Charge-Up Operator is responsible for the correct and complete performance of all firing operations. This includes but is not limited to: • Following the firing procedure and communicating all firing information to their Underground Shift Supervisor (section 6.4.1) • Ensuring the locking of the magazines and explosive boxes on the explosive LV’s at all times when they are unattended and safekeeping of the keys (section 5.3) • Conduct handling, transport, and charging operations in compliance with procedures and approved charging plans • Ensure safe disposal of defective explosives (section 5.5) • Maintaining the stock rotation system (section 5.4) • Complete all required documentation including the Misfire Book (section 6.8) • Ensuring all explosives movements in and out of the magazine are documented in the appropriate magazine log (section 5.4) • Ensuring housekeeping of the magazines is to standard (section 5.4)
